Our Friends are people living with dementia or memory difficulties, and the family and carers who come along with them. Fill in the form below and we will be in touch to welcome you.

What you will need

  • Details of the person living with dementia or memory difficulties
  • Details of their main carer, if they have one
  • An emergency contact who is not the person living with dementia
